Keep in mind he will continue to summon more Caretakers on his turns, and all have to be dead at the same time for the phase to end. The only way to push the boss into the second phase is to have all Caretakers dead at once, so the faster you can accomplish this, the better. Attacking him directly would be pointless until all four are dealt with, so prioritize taking them down while avoiding the Surgeon's scissor attack.
